Practice of Dentistry; Requires persons who apply for licensure renewal as dentists or dental hygienists to furnish certain information to DOH in dental workforce survey; requires Board of Dentistry to issue nondisciplinary citation & notice for failure to complete survey within specified time; provides notification requirements for citation; requires DOH to serve as coordinating body for purpose of collecting, disseminating, & updating dental workforce data, etc.
