| Project Description: |
| The creation of a Federally Qualified Health Center Provider Service Network, provides the state a unique and qualified alternative to assist Florida in its reform efforts. In addition to being capable of providing the current Medicaid services, the HCPSN, is committed to improving health outcomes with a focus on reducing health disparities, providing a medical home to Medicaid recipients, providing services in a culturally competent environment, promoting vital programs as disease management, faith based outreach, patient education, and promoting the health information technology while providing fiscal responsibility and budget certainty to the Florida Medicaid. |
