The Florida Department of Children and Families reports in its 2001-2002 Annual Report on Homeless Conditions in Florida that over the past three years, Florida's homeless population has grown by over 23%. Northeast Florida is experiencing an ever more dramatic increase. For example, The Emergency Services and Homeless Coalition of Jacksonville reported in 2004 that homelessness increased an astounding 86% since 2000 and an astounding 50% of homeless adults have health problems (including HIV).

Project Description:
River Region urgently needs funding to provide support services for 65 HIV+ individuals each month. This project will be the home for the residents of St John's Horizon House, a permanent supportive housing program for people living with HIV/AIDS. On-site recovery and life-skills meetings will be held each week. In addition, program staff will be available 24 hours per day to address issues that may arise. Individuals will live in their very own apartment and will be eligible for supportive services that include case management, substance abuse/mental health counseling, life-skills groups and linkage to medical care. Research indicates that persons who are able to maintain sobriety and positive social relationships are less likely to return to high-risk behaviors that contribute to the spread of HIV.
