Historically, minority women, specifically African �Americans, have a higher incidence of heart disease/diabetes that often goes untreated until the diseases are more advanced and more costly to treat. By improving access to education/screening for prevention and early identification of heart disease/diabetes, this program would lead to decreased healthcare costs (Medicaid, Medicare and other payor categories including the uninsured) as the higher level of medical utilization needed by women diagnosed in the later stages of the disease process would be avoided. |

Project Description: |
Pilot program to improve access to health screening and improve education/awareness of risks of heart disease/diabetes to minority women (primarily African-American). Provide outreach services such as screening/education to this population by bringing it directly to their communities using a van and designated personnel. |

To improve prevention/early identification of risk for heart disease/diabetes. To decrease the incidence of heart disease/diabetes in this population. Decrease the cost of healthcare for treatment of acute/chronic disease that is required when the signs/symptoms are detected later in the disease process. Improve the quality of life of patients by detection and treatment earlier in the disease process. |
