Managed Care Organizations; revises provisions re provider contracts; provides for certain disclosures & requires notice; requires procedures for requesting & granting authorization for utilization of services; provides for HMO liability for payment for services rendered to subscribers; prohibits provider billing of subscribers under specified circumstances; defines term "clean claim"; requires DOI to adopt rules to establish claim form, etc. Amends FS.
