End-of-Life Care; specifies conditions under which hospital personnel may withhold resuscitation; requires use of official form for valid do-not-resuscitate order; adds legislative intent to allow person to plan for future incapacity orally or by executing document; encourages health care professionals to rapidly increase their understanding of end-of-life & palliative health care, etc. Amends Ch. 765, 395.1041, 400.142,.4255,.6095, 401.45, 455.597.
