Health Insurance; Creates "Middle Class and Small Business Free to Choose Act"; authorizes employee of corporation that employs specified number of employees to participate in state group health insurance plan; provides conditions for eligibility to participate in state group health insurance plan; authorizes state resident to participate in state group health insurance plan under certain conditions; requires premium payments to be made electronically; requires corporation to apply for participation in state group health insurance plan, etc.
