Health Care Coverage Procedures; includes preferred provider organizations & health insurers in claim dispute resolution program; specifies timeframes for submission of supporting documentation necessary for dispute resolution; specifies payment of claims provisions applicable to certain health insurers; modifies circumstances under which provider knows that organization is liable for service reimbursement, etc. Amends FS.
