Adverse Incident Notification; amends provision which provides exemption from public records requirements for information contained in notification of adverse incident; removes October 2, 2003, repeal thereof scheduled pursuant to OGSR Act of 1995; provides that such information is discoverable or admissible in disciplinary proceedings by Health Dept. Amends 395.0198.
